🛠️ Fix - Evaluation Forms Save More Reliably After Adding Diagnosis Codes

👥 Audience: Therapists

📅 Date range affected: Intermittent; reported late June 2026

❗ Issue: On some evaluation forms, a clinician could lose unsaved work after adding a diagnosis code and saving. We fixed an issue so the discipline is always available on the form and the document saves correctly.

✅ Action Steps: None. Affected evaluations can be re-entered and saved normally.

🛠️ Fix - Evaluation Forms Keep Entries When Save Fails

👥 Audience: Therapists

📅 Date range affected: Intermittent; reported late June 2026

❗ Issue: If an evaluation didn't save, the page could reload and lose what the clinician had entered. We fixed an issue so the form now stays put with their entries and shows a message, letting them correct it and save without losing work.

✅ Action Steps: None.

🛠️ Fix - Alerts Page Shows Current Alerts by Default

👥 Audience: Therapists, Admins

❗ Issue: The alerts page mixed silenced alerts in with current ones, and very old items could crowd the documentation needing attention lists.

✅ Action Steps: None. The alerts page now shows current alerts by default; use the status filter to view silenced alerts. Organizations can request a date cutoff for their documentation needing attention lists.

🚀 New Feature - Show Only the Selected Toggle Value in Custom Forms

👥 Audience: Therapists / Admins who build Custom Forms

🛠️ Steps to Access: In the form builder, edit a Toggle buttons element and check "Show only the selected value".

📢 What's new?: Toggle button elements now have a "Show only the selected value" option. When turned on, the form output shows only the chosen value for each toggle (hiding the opposite value), keeping notes and reports cleaner.

Thank you for choosing Ambiki.
