How to Access Clinical Documentation In the Patient Portal

How to access clinical documentation in the patient portal 


 

The patient portal now includes a section within the ‘Documents’ tab to share signed clinical documentation including visit notes, evaluations, assessments, plans of care, progress reports, and discharge summaries.  

 

Organizations must first enable this through their EMR settings. Please visit our FAQ to learn how to enable sharing of clinical documentation to the patient portal. 

 

Please note that the patient portal will only show documents that have been signed. It will not show invalidated nor deleted documents. 

 

 

 

To access the clinical documents, ensure you are logged in to the Ambiki patient portal and on the main dashboard: 

  1. In the top tabs click on ‘Documents’.

2. Under the Clinical document PDFs section, you will find all the signed clinical documents. Ambiki will let you know when the document was created, what kind of document it is and the name of the therapist that wrote the document.

3. To view a clinical document in your internet browser, click on the button labelled ‘View’ beside it.

4. You also have the option to download the document in PDF format. Click on the button labelled ‘Download’ to do this.
