Begin a new patient record, entering at minimum the patient guardian’s name and email, then click Save.
The patient or legal guardian fills out a custom intake document form.
The completed intake document is uploaded by the patient/legal guardian.
The system automatically extracts the marked field values from the uploaded intake document. For now, this only includes the following fields: patient sex, DOB, name, and time zone.
These extracted values are automatically populated into the corresponding fields in the patient profile.
The administrator then reviews the imported profile data, makes any corrections needed.
The information does not need to be manually entered twice - it is carried over directly from the submitted intake document to the patient profile.
