How Intake Documents Auto-Fill Patient Profiles

 
Ambiki streamlines patient profile creation by allowing intake documents—submitted by patients or their legal guardians—to automatically populate key fields in a new profile. While a profile must first be initiated by an admin, uploading a completed intake form during setup enables the system to scan and auto-fill details like patient name, date of birth, patient sex, and time zone. This eliminates the need to manually re-enter data, reducing administrative burden and minimizing entry errors.

 
Q: How does the intake document auto-import work when creating a new patient profile? 
 
A: Here is the process when an admin is creating a new patient profile in Ambiki: 

  •  Begin a new patient record, entering at minimum the patient guardian’s name and email, then click Save
 
  • The patient or legal guardian fills out a custom intake document form.  
 
  • The completed intake document is uploaded by the patient/legal guardian. 
 
  • The system automatically extracts the marked field values from the uploaded intake document. For now, this only includes the following fields: patient sex, DOB, name, and time zone
 
  • These extracted values are automatically populated into the corresponding fields in the patient profile. 
 
  • The administrator then reviews the imported profile data, makes any corrections needed. 
 
  • The information does not need to be manually entered twice - it is carried over directly from the submitted intake document to the patient profile.
